Annual Giving Recognition on the Tree of Hope

All contributions, regardless of size, are valued at San Juan Medical Foundation and are entitled to an appropriate acknowledgement and expression of thanks.
Gifts given annually to San Juan Medical Foundation will be recognized in the following manner:

$5,000 Level

individual annual donors making contributions of $5,000 more in the most recent calendar year, independent of planned gifts, or specific campaign donations
corporate and foundation partners making contributions of $25,000 or more in the most recent calendar year, independent of planned gifts, or specific campaign donations

name listed on the Foundation’s website, in the annual report and other publications as appropriate
invitation to a special event recognizing donors
personal visit/invitation for private tour/lunch
recognition of the gift with a copper stone on the Tree of Hope giving display located inside the main entrance to San Juan Regional Medical Center
complimentary premium gift

$1,000 Level

individual annual donors making contributions of $1,000 to $4,999 in the most recent fiscal year, independent of planned gifts, or specific campaign donations
corporate and foundation partners making contributions of $10,000 to $24,999 in the most recent fiscal year, independent of planned gifts, or specific campaign donations
